RULE 11. EVALUATION OF PROGRAM

1. Evaluation Forms:    In order to evaluate the effectiveness of this pilot program, the participants and mediators shall submit to the Trial Court Administrator’s Office completed, questionnaire forms within ten (10) days of conclusion of the mediation session(s). Attached to these rules are evaluation forms labeled Appendix E, Appendix F, and Appendix G respectively for that purpose.



2. Review by Committee:    The “Ad Hoc Committee to Explore Mediation of Financial Issues in Domestic Relations Cases” shall meet periodically to monitor the program and report annually to the Chief Judge.
